Bundamedik Transformation Creates Digital Chain of Health Services

Bundamedik Healthcare System (BHMS) is an Indonesian health service provider company that is actively exploring the use of modern medical technology. Bundamedik Starting from the maternity clinic which was founded in 1973 by Dr. Rizal Sini, father of dr. Ivan Rizal Sini who currently holds the position of Main Commissioner of Bundamedik.

Bundamedik then developed into the owner of the Bunda Mother and Child Hospital (RSIA) network and the Diagnostic Laboratory. The company is quite active in exploring health innovations through investing in several startups healthtech, namely Smart Clinic, Asa Ren, and Nalagenetics. Dr. Ivan is also known to be one of the Founders Moosa Genetics, a biotechnology startup for the livestock industry.

In his capacity as President Commissioner, dr. Ivan Rizal Here explains the ins and outs of the health industry's macro challenges and efforts transformsito address micro issues customer journey patient.

Health industry issues

The latest data collected Central Bureau of Statistics (BPS) In 2022, there will be 176.110 doctors in Indonesia, including general practitioners, specialist doctors, dentists and specialist dentists. However, this figure does not meet WHO's ideal standards, which set a ratio of 1 doctor per 1.000 population. The ratio of doctors in Indonesia is only 0,63 doctors per 1.000 population.

Limited resources in the health industry have long been one of a series of issues in the health industry apart from issues of accessibility and affordability. However, the gap in this issue was acknowledged by Dr. Ivan only really realized it when the Covid-19 pandemic broke out three years ago.

"Micro challenges will not be resolved if macro issues are not resolved first. I appreciate what the government is doing to identify needs healthcare based on population problems. With the digital transformation of the [Ministry of Health] at the macro level, we can talk about what can besolves at the micro level," he told DailySocial.id.

In his description, the health industry is a long service chain, including aspects of education, screening methods, early detection, curative treatment, and palliative treatment. However, he believes that most hospitals focus on curative and palliative treatment. Not much goes to the educational level and is preventive in nature. As a result, many patients come when they are sick. Health facilities are not involved at much level screening methods to identify patient needs.

"We cannot tell what's actually their needs. People don't know what it actually takes to make it new requests. As far as we can make it demand new. At the moment, the gap is just too big for us to decide [How far we have gone in resolving this issue], that's just up to here. "There are many diseases, we haven't talked about education, the service process in hospitals, and the speed of diagnosis that need to be improved," he said.

This also makes disruption in the health industry not as easy as other sectors, such as financial services or transportation. Health services have high complexity because they cover various types of diseases, health data and patient needs. The regulations are very strict because they are closely related to human life.

As an illustration, the disruption of health services in Indonesia is still mostly focused on telemedicine, either developed by health facilities or in partnership with platform owners, such as Halodoc and Alodokter. Even though it appeared before the pandemic, its use has only become massive in the last 3-4 years due to the urgency of handling Covid-19. Indonesian people mostly use telemedicine to consult doctors (40%) and buy medicine (30%) according to Deloitte survey in 2022.

Bundamedik digitalization

By analyzing various issues in the health industry, Bundamedik seeks to transform health services, not only based on the urgency of need, but also looking at the economic scale.

"In the last 5-10 years, we have explored a lot of the need to improve processes in clinical services. We have unified all systems into one platform for the past 5 years. We don't think about getting in the early stage of innovation because Bundamedik has many initiatives so that they can complement each other. Improvements in internal processes are very important through digitalization."

From the service aspect, dr. Ivan mapped out several problems that patients often experience from the start until they seek treatment. For example, waiting time at the hospital. According to him, the waiting time could be much longer than expected due to unexpected factors, such as sudden surgery. Another example of an issue is the difficulty of scheduling a consultation with a doctor.

Bundamedik has formed a special Digital Transformation and Customer Engagement (DTCE) division which will function to evaluate customer journey the patient becomes digital Journey, including overcoming problems faced by patients. The results are translated via the OneBunda platform which will launch in March 2023. It is claimed that the OneBunda application has been used by more than 65.000 Bundamedik patients.

Currently, patients carry out teleconsultations and make consultation schedules, including child vaccinations. To increase journey patients, it plans to connect OneBunda as entry point with third parties in the health ecosystem

OneBunda was developed as channeling platform which will be connected to Bundamedik's ecosystem and other initiatives. Apart from the hospital network, Bundamedik oversees the Morula IVF program and the Diagnostic Lab. This integration allows Bundamedik to open access to one pintu to patients who have customer journey different.

"We want to open the API to various parties who focus on customer journey and clinical services, but in stages. This will help us maintain patient retention rates. With digitalization, we know how much dropped out from patients who come to Bundamedik. This is important for evaluating patients who have high or high return rates churn rate high.

"Connecting OneBunda to this health ecosystem will make it easier for clients or patients at Bundamedik. They can have better health services personalized," added Dr. Ivan.

In his position as Chair of the Indonesian Genomics Association (AGI), he also spoke about the potential of BGSi in encouraging the emergence of new startups which might focus on certain chronic diseases, such as diabetes or lung cancer. They can utilize the data collected by BGSi without needing to collect samples from the start.

BGSi or Biomedical & Genome Science Initiative is the Ministry of Health's first initiative to develop appropriate treatment methods for the community. They are targeting collection of 100 thousand samples in 2025 to map the genome data. This sample collection is also an important basis for producing relevant and accurate tests because it uses Indonesia's own population base.

Not only for human needs, genomics can also be applied to plants and animals. Dr. Ivan gave an example of how genomic tests allow us to identify the rice profile that is most resistant to pests, or the most suitable fertilizer.

"That's why I am very enthusiastic about inviting my friends to build AGI, because this must be able to represent industry. Not just from a science perspective, but how industry works. healthcare whose basis at the genomic level can be developed. "This [association] must be inclusive, anyone can join."

Currently, AGI has almost 70 members and more than 15 institutions.
