Administrative requirements for prospective individual agents:

• Copy of ID card
• Copy of NPWP (Taxpayer Identification Number)
• Copy of KK
• Photo of the front page of the passbook
• Photo or coordinates of the location of the place of business 

Administrative requirements for prospective agents in the form of business entities (minimum CV):

• Photocopy of ID of the owner/director
• Photocopy of deed of establishment/change of company
• Photocopy of SIUP (Business License for Company Establishment)
• Photocopy of Company Registration Certificate (TDP) / Certificate of Domicile
• Photocopy of Company NPWP / KTP Confirmation Letter / Registration Certificate of the Director General of Taxes
• Attach the coordinates / photo of the location of the place of business
• Have a place of business as stated on the registration form (minimum 2 years for a business place that is still rented)
• Photocopy of the first page of the account book

Operational Terms

• Minimum 1 PC or laptop
• Printer (preferably thermal printer)
